**BEFORE STARTING, Please download the W-9 form, complete the form and upload a PDF or photo of it below.**

You will also need a copy of your General Liability Insurance Policy & Workers Comp Insurance Policy OR (if applicable) a Certificate of Non-Coverage. (Only applicable if you are a sole proprietor with no employees).  A PDF is preferable, but a picture will suffice.
